WHAT TO EXPECT

WHAT WE DO

Serious eye problems can occur to anyone at any time. You can sustain an eye injury during an accident or while playing sports; you may develop an eye disease as you age, or you may simply be born with a pediatric eye condition that requires surgical attention. Whatever the reason for your impaired vision, Arizona Eye Specialists has an experienced team in place to diagnose and treat your vision problem. We work hard every day to save our patients’ vision and help them lead fuller lives.

PATIENT PORTAL

Arizona Eye Specialists has launched our PATIENT PORTAL for your convenience. For existing patients to register, go to the right-hand side of the portal and click on First Time Patient Portal Access, then CONTINUE. When you register for the first time, you will need an ACTIVATION KEY that you will receive from our offices or by contacting us at 480-994-5012. The Patient Portal is currently for existing patients ONLY.

PATIENT FORMS

When you arrive, you will be greeted by our medical receptionist who will kindly ask you to complete the patient forms packet.

You can save time in the office by completing BOTH required forms marked in RED before your appointment. These are two (2) separate forms so be sure you fill out each one and print both of them and bring them with you to your appointment.

These forms can be filled out online, then printed to bring with you to your appointment. If you’d prefer, you can print them, then complete them by hand.

WHAT TO BRING

Please bring the following items with you to your appointment:

  • Both completed patient forms – Patient Registration Form AND Medical History Questionnaire
  • Medical records from your referring doctor, if available
  • A list of all your current medications and their dosages (Please include over-the-counter medications as well as Rx.)
  • Identification with your photograph
  • Your insurance card
  • Your glasses and/or contact lenses
  • A pair of dark sunglasses to protect your dilated eyes after your appointment.

YOUR VISIT

If you are a new patient, please plan to arrive at least 15 minutes early to help ensure your records have the important information we need. We know things come up unexpectedly and we understand. Arizona Eye Specialists requires 24-hour advance notice to avoid any cancellation fees.

Your initial appointment will last between 1 and 3 hours as we may conduct many tests during your visit. This will help us determine the health of your eye and provide treatment options.

Your medical evaluation will begin with an ophthalmic technician who will talk with you in order to get a more detailed medical and ocular history. You will be questioned regarding your visual symptoms and the treatments you have received. The technician will then place drops in your eyes to dilate (widen) your pupils. Dilation is necessary for a complete examination of the eye. Your pupils may require up to 30 minutes to dilate. Since your pupils may remain dilated for a full day after your visit, we recommend that you bring sunglasses to reduce your sensitivity to light. Your ability to read will be temporarily decreased. We also recommend you have someone drive you home.

After your pupils are dilated, one of our doctors will meet with you to examine your eyes. Depending on your medical condition, other specific tests may be performed.

If treatment of your eye problem is indicated, you and your Arizona Eye Specialists doctor will decide the most appropriate course of action. Some types of treatment can be performed in our office and others require a procedure in a surgery center.

INSURANCE & PAYMENTS

Our physicians accept multiple different health plans. You can refer to our INSURANCE INFORMATION.  If you don’t see your plan listed, feel free to contact us at 480-994-5012. Please check with your appointment scheduler as all doctors are not contracted with all the insurance carriers.

Our offices accept VISA, MasterCard, Discover, American Express, personal checks, and cash.

If you would like to pay by phone, call 480-994-5012, Option 7. If you prefer to mail your payment, please enclose a check, money order or credit card information with the payment stub from your statement in the enclosed envelope. To pay your bill online, click PAY NOW, then enter your name and account number from your statement. You will then be redirected to a secure page to enter your payment information. There are NO fees to pay your bill online.
