I am writing to let you know that as of August 17, 2018, I have retired from Arizona Eye Specialists. The decision to retire was a difficult one because I truly enjoy my relationships with my patients and staff and have gotten endless satisfaction and joy from caring for you over the past 18 years. It has been an honor to have served you. Thank you for your kindness, your loyalty, and your trust.
The same friendly and caring staff at Arizona Eye Specialists will continue to serve you, and as always, you will be contacted for your periodic eye exams. As you all know, this practice is staffed with some of the most knowledgeable and caring physicians in the state, several of whom have already graciously offered to assist you with your eye care needs. I know you will find that you’ll receive the same outstanding service you are accustomed to. I encourage you to continue your eye care at Arizona Eye Specialists.
It is very important that you continue to receive eye care. Not doing so can result in a delay in the current treatment you are receiving and a delay in detecting other eye conditions that can negatively impact your vision.
We understand that some of you may want to select a new physician outside of our practice. Please be assured the staff at Arizona Eye Specialists will do everything they can to make the transition smooth and stress-free.
Your medical records are confidential and will remain on file with Arizona Eye Specialists. If you choose another physician within our organization, you do not need to take any action concerning your records. If you choose a physician outside of our practice, you will need to sign an authorization form before we can release your medical records. Please note, by law, we cannot share your medical information without your written consent. Should you elect to seek eye care elsewhere, please contact our medical records department at 480-994-5012 ext. 8. They will be more than happy to assist you in obtaining your personal records.
I want to thank each of you for the opportunity to be your eye doctor and for your past and present support. It means more to me than I could ever express. Please know that you have my best wishes for the future.
